Enumeration is the card's own responsibility, and this card has stopped doing it. When media is inserted, its controller must wake, identify itself and declare a capacity before any host shows it. "No SD card inserted" from a phone holding one is the handset reporting silence from the slot — the electronic equivalent of knocking on a door nobody answers.
The reader appearing without the card is a detail that misleads almost everyone. A USB card reader is its own small device: the laptop sees the reader's bridge and lists it, medium or no medium. The entry that showed up was the empty doorway, not the card — which is why clicking it produced nothing and why the card's absence from disk management is the truthful reading.
